"установленной формы" means here not only the form itself or the document in the presribed form but also the fact that the document is duly executed in accordance with the form prescribed by legislation ("по установленной форме");
hence = duly executed,
which, for me, sounds better than
"in accordance with a set form” or “in the prescribed form”

Однако почему-то предпочитают form. Вспомните, к примеру, распространенную формулировку "substantially in the form attached hereto as Exhibit..."
