Исследование выполнено в режимах Т1, Т2 и STIR ВИ.
На серии МР томограмм в проекции межкостного пространства, распространяясь вдоль малоберцовой и большеберцовой костей с в/3 голени до дистального межберцового синдесмоза определяется сосудистая мальформация с явлениями седиментации. На уровне с/3 голени по передней поверхности мальформация распространяется в подкожную жировую клетчатку.
ЗАКЛЮЧЕНИЕ: МР-картина наиболее всего соответствует артериовенозной мальформации сосудов левой голени.

MRI scans performed in T1, T2, and STIR WI modes
The serial MRI scans of the interosseous space view are determining a vascular malformation with signs of sedimentation extending along the fibula and tibia from the upper third of the shin to the distal tibiofibular articulation. At the level of the middle third of the shin along the anterior surface the malformation is extending into the subcutaneous fat.
CONCLUSION: The MR picture is corresponding to the arteriovenous malformation of the vessels in the left shin.

AGREEMENT
I, {Name} (“the Owners”) in our capacity as the Beneficial Owners of {Company Name}, a Company incorporated under the laws of Cyprus on {Date}under number {...} hereby request {Company Name} (“{Short Company Name}” and which term shall include each person or company nominated by {Short Company Name} who may from time to time acted according to instructions as director, alternate director, secretary, assistance secretary, manager or other officer or registered shareholder of the Company) to proceed with the necessary formalities for the strike off of the entity of the Registry of Cyprus.
The Owners will, at all times hereafter indemnify and keep indemnified {Short Company Name} from and against all actions, suits proceedings, claims, demands, costs, expenses and liabilities whatsoever which may arise or occur or to be taken, commenced, made or sought from or against {Short Company Name} in connection with, or arising from, any Services rendered in the past or to be rendered for the strike off of the entities save and except for any fraudulent or grossly negligent act or omission on the part of {Short Company Name}.
The Owners guarantee the due payment to {Short Company Name} of all fees, disbursements and expenses in connection with any Services rendered in the past or to be rendered for the closing down of the entities. In addition, the Owners guarantee that the Company is no longer carrying on any business and it has neither assets nor liabilities whatsoever and that the Company does not intend to carry on business in the future. Furthermore, the Owners guarantee that all the relevant bank accounts, which the Company might have had, have been closed.

Until instructed in writing to the contrary all instructions to be given to Trident pursuant to the Agreement shall be given to {Company Name} of {Company Address}
This Agreement shall be governed by and construed in accordance with the laws of Cyprus and the parties hereto submit to the non-exclusive jurisdiction of the courts of Cyprus in connection herewith.

Собственники гарантируют {Краткое название компании} своевременную оплату услуг, а также всех сборов, накладных расходов, понесенных в связи с любыми услугами, оказанными в прошлом или предоставляемых в связи с закрытием компаний. Кроме того Собственники гарантируют, что компания более не ведет никакой деловой активности и не имеет активов и обязательств какого-либо рода и что компания не имеет намерений вести какую-либо деловую активность в будущем. Более того, Собственники гарантируют, что все счета компании закрыты.

Если не поступит иного письменного указания, все инструкции и указания, данные {Краткое название компании} по исполнению настоящего соглашения будут направляться в {Название компании} по адресу: {Адрес}.
Настоящее соглашение регулируется законодательством Кипра, подлежит толкованию и приведению в исполнение в соответствии с ним, и стороны установили, что все вопросы, проистекающие в связи с исполнением данного соглашения, относятся к неисключительной юрисдикции судов Кипра.
Настоящим подтверждаем получение настоящего соглашения, прочтенного и понятого нами, выражаем свое согласие приступить к действиям, необходимым для административного исключения компании из реестра юридических лиц Кипра.
