Network Cable and Network Connection
The PRI (Primary Rate Interface) network interface module is designed to be CPE (Customer Premise Equipment).This means the PRI network interface module does not generate a clock signal to the network. Instead, the module derives the clock from the upstream switch or from the PBX to which it is connected. This is sometimes referred to as slave mode.
Note 1: The term upstream refers to network devices that are closer to the telephone service provider. The term downstream refers to network devices that are closer to some video communications unit. For example, the PRI network interface module might be downstream from your PBX, which, in turn, is downstream from a telephone company switch. Your PBX is upstream from the PRI network interface module.
SNAPSHOT
You can send a snapshot from any local camera to the participants in a videoconference by using the snapshot feature on your videoconference system.
Snapshot Timeout
By default, all slides and snapshots are displayed for a period of four minutes. However, when this option is disabled, the snapshot or slide stays on screen indefinitely until the user presses the snapshot button on the remote control to return to live video.
To disable this option:
Select System Info>Admin Setup>Video/Camera>TV Monitors.
In the TV Monitors screen, deselect Snapshot Timeout.
MANAGE IDENTITIES
Identities are different subsets of passwords from which you can seamlessly switch back and forth. You can create an identity called “home” or “work”, anything that will help you to manage your passwords.
Switch Identities
To switch identities at any time, right click on the key-shaped icon on the taskbar and choose Switch User Identity. The logon user screen will pop up. Choose the desired identity from the drop-down menu and choose O.K.

DESCRIPTION
The PIP Fixation Hinge is an external fixation device designed to control rotation of an injured finger to regain, maintain or increase the range of motion of the proximal interphalangeal (PIP) joint. It utilizes small diameter wire fixation techniques and procedures for placement and is intended to be centered on the axis of rotation. The device is designed to permit adjustment on the axis of rotation. Consult surgical technique for recommended procedures. Composition of the device permits radiographic visualization
During treatment the patient must adjust the device. Under advice from the surgeon, the patient must turn a knob that moves the phalanges through flexion and extension of the PIP joint. The knob can be retracted to allow disengagement from the gear. With the worm disengaged, the two braces slide freely permitting active motion for rehabilitation. The PIP Fixation Hinge is made from plastic and metal materials. The component material is provided on the outside carton label. Do not mix components from different manufacturers. All components are designed for single use only.
INDICATIONS
1. Post-traumatic PIP joint contracture which has resulted in lost range of motion.
2. Fractures and disease of the fingers, PIP joints and hands which generally may result in joint contractures or loss of range of motion.
CONTRAINDICATION
The device is contraindicated for use in uncooperative or mentally incompetent patients who are unable to follow the postoperative regimen.
WARNINGS
1. Wire placement requires strict anatomic considerations to avoid damage to nerves or blood vessels. The wire should be gently pushed through soft tissue, not drilled, to reduce possibility of injury.
2. Wire drilling in the bone should be done slowly to avoid heat necrosis of surrounding tissue and bone.
3. Use caution when handling the sharp tips of the wires. The tip of the wire should be held when clipped. Eye protection is recommended for operating room personnel.
4. Wire site care is crucial in reducing localized infections.
5. Intraoperative fracture or instrument breakage can occur. Instruments which have been used extensively or with excessive force are susceptible to fracture. Examine all instruments for wear and damage prior to surgery. Replace where necessary.

Текст оригинала - английский Lipoatrophy is usually a complication of T1DM treatment. Lipoatrophy was commonly seen with old insulins (bovine and porcine). It is now rarely observed since the introduction of highly purified insulins and insulin analogues.
In 2005, Ampudia-Blasco reported the case of a 39-year-old patient with T1DM who developed lipoatrophy at the injections sites with insulin Glargine.1 Most of the other previously described cases were associated with the use of insulin Lispro. A 10-year-old Caucasian girl from Yale University developed localised lipoatrophy of the subcutaneous tissue of the anterior abdominal wall a year after treatment with continuous subcutaneous insulin infusion (CSII) using Lispro insulin.2 The lipoatrophy did not progress further after switching to buffered human regular insulin using the same MiniMed CSII pump. Therefore, the authors attributed the cause to Lispro insulin. However, lipoatrophy was also reported with CSII which did not regress after changing Lispro to buffered human insulin.3 There was local hyperproduction of tumour necrosis factor α from macrophages leading to differentiation of adipocytes resulting in lipoatrophy. It was therefore hypothesised that Teflon cannulae used in the pump system may have induced the local immune reaction.3 Another case was reported with lipoatrophy induced by Lispro insulin but not in combination with pump therapy.4 Most recently, a 13-year-old girl with injection-site lipoatrophy was described, also in association with Lispro insulin.5
The pathogenesis of lipoatrophy is still poorly understood. The suggested mechanisms are: repeated mechanical trauma from the injections, cryotrauma from refrigerated insulin or immune mediation. Many studies have demonstrated an association between high insulin antibodies and lipoatrophy. In a study conducted at the diabetes clinic of the University Children's Hospital in Munich, Germany, 112 children were randomly recruited for measurement of insulin antibodies.6 Lipoatrophy was found in four children who had significantly high insulin antibody levels. This study concluded that there is a strong association between insulin antibody levels and lipoatrophy, which implies immunity against insulin is a factor in the development of lipoatrophy. Another report in 1998 also described a significantly high insulin antibody level in an adult with diabetes.7 We observed high levels of insulin antibody in three of our cases.
The management of lipoatrophy has not yet been well established. Suggested treatments include injecting insulin around the atrophic sites in combination with dexamethasone.8 In two of our patients, lipoatrophy resolved after changing the injection sites alone, suggesting that local factors could be contributing to the development of lipoatrophy. However, in the other two children lipoatrophy developed at the new injection sites but resolved after we changed the insulin preparations. This suggests that immunity against insulin might be a contributing factor. HbA1c decreased after resolution in two of our patients but remained the same in other two. HbA1c decreased in only one of the two patients where lipoatrophy resolved after changing sites, suggesting that poor absorption from the site with lipoatrophy might be contributing to the high HbA1c levels in at least one of those patients. In the other two patients whose lipoatrophy resolved only after changing insulin preparation, HbA1c improved in only one, suggesting that immunity might have caused poor insulin absorption from the local lipoatrophy site.

TECHNIQUES FOR TREATING MULTIPLEWOUNDS
Y-connector technique
By applying a Y-connector to the canister tubing, one V.A.C.®Therapy Unit may be used to simultaneously treat multiple wounds on the same patient. If this technique is used, all dressed wound sites must be assessed for seal integrity. The dressing should be collapsed. V.A.C.®GranuFoam® and V.A.C. GranuFoam Silver® Dressings should have a wrinkled appearance. There should be no hissing sounds.
- SensaT.R.A.C.®/T.R.A.C.® Technology only senses one wound site, the side with the post (male port), even when multiple sites are being treated.
- Do not connect infected wounds with non-infected wounds through a Y-connector.
- Do not connect wounds with different etiology in which cross contamination may occur.
-Avoid using a Y-connector to connect wounds that would be optimally treated with differing pressure settings.
-Consider the Y-connector as an extension of canister tubing.
Change the Y-connector at least once a week or more frequently, as indicated, when the canister is changed. Dispose of the Y-connector, the canister tubing and the canister in accordance with specific institution or State Regulations.
Bridging technique
Wounds that are in close proximity to one another on the same
patient and of similar pathologies may also be treated with one V.A.C.® Therapy Unit using a technique known as bridging. The advantage of bridging is that it requires only one piece of tubing, which may be more convenient for the patient.
Step-by-step bridging guidelines:
1. Protect intact skin between the two wounds with a piece of V.A.C.® Drape or other skin barrier such as a hydrocolloid dressing or a vapor-permeable adhesive film dressing.
2. Place foam dressing in both wounds, then connect the two wounds with an additional piece of foam, forming a bridge. All foam pieces must be in direct contact with each other.
3. It is important to place the SensaT.R.A.C.®/T.R.A.C.® Pad in a central location to ensure that exudate from one wound is not drawn across the other wound.
4. It is not recommended to bridge wounds of different etiologies or to bridge an infected wound to a non-infected wound.
TECHNIQUES FOR TUNNELING AND SINUS TRACTS
V.A.C.® WhiteFoam Dressing is recommended for use in tunnels. Always cut the V.A.C.® WhiteFoam Dressing wider at one end and narrow at the other. This will ensure that the opening to the tunnel or sinus tract remains patent until the distal portion of the tunnel has closed. Continuous therapy should always be used until the tunnel has completely closed.
Do not place foam into blind or unexplored tunnels.
