... are a few fields to add remarks, as well as options to answer "Don't know," "Maybe," "Not applicable," etc.

There were a few questions for which my answers needed to be qualified, in particular the question about "Would I recommend this software to new users?" I selected "Likely" only because it was the least unacceptable answer available
