What happens when wordfast.dot is copied to the Startup folder and you fire up Word? Can you press Ctrl+Alt+W and/or Ctrl+Alt+P? Ctrl+Alt+W (press it twice, just in case) should cause the setup screen to pop up. Ctrl+Alt+P should produce a report with your configuration.
Can you see any other files than wordfast.dot in the Startup folder? The following two files should be created by Wordfast: wordfast.cfg, wordfast.ini
