Although each company's form is different, certain pieces of information tend to appear. A good EOB (explanation of benefits) form has all of the following:
- Information on the policy holder—the employee, if the coverage is provided by a company. This might include name, address, Social Security number, and/or other identifying numbers.
- Information about the person who received medical care—name, relation to covered person, Social Security number, and possibly other identifying numbers.
- The name and address of the health plan.

...siehe auch Leo (http://dict.leo.org/?lp=ende&lang=de&searchLoc=0&cmpType=rel... ), obwohl ich wahrscheinlich eher etwas wie "betrieblicher Gesundheitsplan" oder "betrieblicher Gesundheitsvorsorgeplan" vorgeschlagen hätte. ;-)
