In the Soviet Union (1986!!!) 'military service record books' (MSRBs) were issued to:
- young men who have served their duty in the USSR Armed Forces under conscription;
- graduates of higher military education institutions who have entered them right after graduating from a regular secondary school and completed the training successfully (mostly males);
- graduates (both males and females) of selected civilian universities including medical (where courses of "battlefield surgery" were a 'must' for all).

For example, I was issued a MSRB after graduating from my Institute (now - University) in 1980 with the military rank of 'lieutenant" and since then I have never been troubled by the Armed Forces. My spouse (an M.D.) was issued the same in the mid-1980s after graduating from her medical university (with the same outcome).

And our MSRBs do NOT contain any entries on our "main civilian qualifications" but just the code number of our relevant 'military occupational specialty'.

So, I may only guess that an entry on the "main civilian qualification" is a peculiar feature of MSRBs issued (in 1986) to a 'young man who have served his duty in the USSR Armed Forces under conscription'.
