Fluffy seems a bit more common, but cotton wool-like is obviously closer to the original.

Note the ‘fluffy’ or ‘cotton wool-like’ appearance. Air

perihiliar bilateral alveolar filling pattern.

You may want to use "like clouds or cotton balls". From my particular experience, this may not be as common as alveolar filling pattern.
