As the date is the same as for the attestation, perhaps it is discharge? As the doctor is supporting the patient in his dispute isn't it more likely to have been written after the procedure?

Asker: I agree that from the context we can assume that *in this particular instance* it means "discharge letter". But the link states clearly that, without any context, it can also mean a letter before admission. I tried to make clear that it's an English term covering *both* cases that I'm looking for. It may not exist.
