Tears of the posterior horn of the medial meniscus in the middle aged patient present a challenging clinical problem. As highlighted in the cases presented, clinical decision making in the middle aged individual should be based on degree of associated pathology and patient activity level and goals. Biomechanical studies clearly show that these tears lead to *loss of hoop stress* and extrusion, which profoundly impact the contact pressures within the knee. It’s postulated that this may advance joint degeneration, however long term studies about the natural history of these tears are needed. Clinical outcomes for both non operative treatment and meniscectomy are largely limited to retrospective case series.
