You can be sure that it is indeed Diagnosis. First, look at the entries for Dx in Cosnautas (our trusted, go-to, best online resource for Spanish medical abbreviations and acronyms):
1 diagnóstico {→ Dco. 1} 2 diagonal 3 dextrosa {→ G 26}
Also Ian Beattie's exhaustive hardcover Medical Abbreviations - Spanish/English book shows Dx = diagnóstico.
Nobody shows Dx = dirección, and as Maria said ... there is no address given.
From the little that our asker gives us (we could have used a bit more background) it only makes sense that this is the admitting diagnosis (they don't always give a specific disease/disorder as the diagnosis), the reason for the referral.
I think the asker can easily make the decision.
