脳神経では、眼球運動full、眼振、複視なし。対光反射迅速、顔面感覚左右差なし。
顔面神経麻痺なし。聴力は左右差はないものの、音の刺激でめまい感の出現はありました。軟口蓋挙上可能で挺舌正中でした。
運動は、四肢麻痺なく、深部腱反射正常、Babinski徴候陰性。
感覚も、触覚、振動覚ともに鈍麻なし。
協調運動は指鼻指試験、膝踵試験ともにスムーズで失調なし。
歩行は可能で、つぎ足歩行も何とか可能でした。
診察上は、小脳失調や深部覚障害はない点、耳鳴、耳痛があることから耳性のめまいの可能性があるのではないかと考える旨を御説明致しました。
当院耳鼻科に紹介受診頂き、その際での診察上は鼓膜所見などは正常であったようです。頭部MRI及び側頭骨CT施行され、明らかな異常所見は指摘されておりません。更なる検査に関して、現時点では当院では難しい可能性を御説町致しました。
以前受診歴のある横須賀の耳鼻咽喉科医と協議し、今後の方針と検討していきたいというご希望でした。
Перевод - английский Based on the medical examination –
Regarding the cranial nerves, there was the full eye movement observed, no nystagmus, and no diplopia. There was a rapid light reflex, and no difference in facial sensation between the left and right sides.
No facial nerve paralysis. Although there was no difference in hearing between the left and right ears, the patient experienced dizziness due to sound stimulation. With the soft palate lifted up, the tongue was midline.
The movements were without quadriplegia, tendon reflex was normal, and there was a negative Babinski response.
No loss of sensation, either tactile or vibratory.
Coordinated movements were smooth in both the finger-nose test and the heel-to-shin test, and there was no ataxia.
The patient was able to walk and to perform the tandem gait test.
During the examination, the patient was explained that there was no cerebellar ataxia or deep sensibility disorder, but dizziness in the ears might be caused by tinnitus and ear pain.
The patient was referred to the otolaryngology department in our clinic, and it seems that the eardrum examination results were normal at that time. A head MRI and temporal bone CT scan were performed as well. There were no obvious abnormal findings. Regarding further examinations, the patient was explained that it may be difficult to conduct at our hospital.
The patient expressed her desire to consult with an ENT doctor in Yokosuka, whom she had previously visited, and consider the following course of treatment.

Malignant glioma with FOXR2 activation, after verification of the histopathological diagnosis in Germany - CNS Neuroblastoma with FOXR2 activation WHO grade-3 01/22/2021, after partial resection of brain tumor, 11/22/2017, with disease progression - MRI scan 05/16/2018, treated with Temodal chemotherapy 09/10/2018 -08/2020, after radiotherapy 54.4 Gy 05/22/2018 – 07/03/2018, was admitted to the Chemotherapy Ward of KPHiO [Pediatrics, Hematology and Oncology Clinic] on 02/28/2020 for oncological monitoring
(including MRI scan of the brain and spinal canal under general anesthesia). Last follow-up MRI 01/19/2023 - stabilization of the disease.
A follow-up MRI of the brain and spinal canal with contrast under general anesthesia was performed on 08/10/2023. In the description of the examination, the whole image of the brain and the bed from the removed tumor stable compared to the previous examination. Lung ultrasound did not show features of pneumothorax. The child was discharged home today in good condition.

Злоякісна гліома з активацією FOXR2, після верифікації гістопатологічного діагнозу в Німеччині - нейробластома ЦНС з активацією FOXR2 3-го ступеня ВООЗ від 22/01/2021, після часткової резекції пухлини мозку від 22/11/2017, з прогресуванням хвороби - МРТ скан від 16/05/2018, лікувався за допомогою хімієтерапії темодалом у період 10/09/2018 -08/2020, після променевої терапії 54,4 Гр у період 22/05/2018 – 03/07/2018, 28/02/2020 був госпіталізований до відділення хімієтерапії KPHiO [Клініка педіатрії, гематології та онкології] для онкологічного моніторингу (включаючи МРТ головного мозку та спинномозкового каналу під загальним наркозом). Останнє контрольне МРТ було зроблено 19/01/2023 - стабілізація хвороби.
Проведено контрольне МРТ головного мозку та спинномозкового каналу з контрастуванням під загальним наркозом 10/08/2023. В описі обстеження, повне зображення головного мозку та ложа від видаленої пухлини стабільне порівняно з попереднім обстеженням. УЗД легенів не виявило ознак пневмотораксу. Сьогодні дитину виписали додому в задовільному стані.

The product has been evaluated for safety in 30 clinical trials involving 5000 patients. Of these 5000 patients, 3000 patients were treated with the product monotherapy, including 1000 patients treated for ≥ 5 years and 2000 patients treated in placebo-controlled trials.
The following potentially serious adverse events have been reported rarely with the product in controlled clinical trials: syncope and hypotension. In placebo-controlled trials, no serious adverse event was reported with a frequency ≥ 0.01% in product-treated patients.
The adverse drug events listed below have been accumulated from 30 clinical trials including 5000 hypertensive patients treated with the product. Adverse events have been ranked under headings of frequency using the following convention: very common (≥ 1/10); common (≥ 1/100, < 1/10); uncommon (≥ 1/1000,
