The initial rhythm of the patient was pulseless electrical activity (PEA).
For this reason I made the decision to perform a thoracotomy, the opening of the chest, in order to identify the point of blood loss and stop it.
I was assisted in this procedure by Dr [XY].
During the procedure, the Cardiothoracic Surgeons were also contacted and arrived.
As a result of the thoracotomy, it became evident that the point of blood loss was at the right hilum of the lung and this was clamped with a surgical clamp and gauze with the right lung compressed within an inco-sheet.
