I remember reading once that some fellows use language to conceal thought, but it's been my experience that a good many more use it instead of thought.
A businessman's conversation should be regulated by fewer and simpler rules than any other function of the human animal. They are:
Have something to say.
Say it.
Stop talking.
Beginning before you know what you want to say and keeping on after you have said it lands a merchant in a lawsuit or the poorhouse, and the first is a short cut to the second. I maintain a legal department here, and it costs a lot of money, but it's to keep me from going to law.
It's all right when you are calling on a girl or talking with friends after dinner to run a conversation like a Sunday-school excursion, with stops to pick flowers; but in the office your sentences should be the shortest distance possible between periods. Cut out the introduction and the peroration, and stop before you get to secondly. You've got to preach short sermons to catch sinners; and deacons won't believe they need long ones themselves. Combined Analysis of the Incidence of Adverse Events
In the 4 vehicle-controlled studies, the discontinuation rates ranged from 9.4% to 25% in the XXX groups and from 29% to 48% in the vehicle groups. The main reason for this imbalance was the higher incidence of premature discontinuations due to unsatisfactory therapeutic effect in the vehicle groups (range: 14% to 41%) in comparison with the XXX groups (range: 0 to 10%). To account for the time on studies and multiplicity of adverse events of the same type, the total number of event occurrences (i.e., events count) during the studies were analyzed using the incidence density (ID) rate of all adverse events. Data from all studies were pooled for this analysis to provide the largest available dataset of adverse events.
Most Frequently Reported Adverse Events
The incidence density (ID) rates for adverse events that occurred in more than 1% of patients in any group are presented in Table 1. The majority of the most frequently reported adverse events were common childhood disorders such as nasopharyngitis, pyrexia, upper respiratory tract infection, ear infection and bronchitis. The ID rate for all adverse events was similar in the XXX and Vehicle groups, except for teething (discomfort experienced during the eruption of teeth through the gums), which was observed more frequently in patients treated with XXX than in patients treated with the vehicle (relative risk: 2.02; 95% CI: 1.32-3.27; P = 0.002).

По всем явлениям отмечена одинаковая ПС в группах XXX и плацебо за исключением прорезывания зубов (возникающего при этом дискомфорта), чаще наблюдавшегося у пациентов, получавших XXX, по сравнению с получавшими плацебо (относительный риск: 2,02; ДИ на уровне значимости 95% 1,32-3,27; P = 0,002).

Больной Е., 47 лет. Диагноз: ИБС, стенокардия напряжения III функционального класса. Давящие боли за грудиной, иррадиирующие в левое плечо, возникают ежедневно при ходьбе, подъеме по лестнице. Боли в течение 2-3 мин. проходят после прекращения движения или приема нитроглицерина. При коронарографическом исследовании выявлен 90% стеноз правой коронарной артерии и 50% стеноз огибающей ветви левой коронарной артерии. 27.06.85 проведена велоэргометрическая проба. В покое, до проведения пробы, патологические изменения ЭКГ не выявлены.
При проведении велоэргометрической пробы до "пассивного" курения толерантность к нагрузке составила 75 Вт, ДП - 220 усл. ед., ДП/Вт - 2.9. Причина прекращения велоэргометрической пробы - приступ стенокардии и смещение сегмента ST в 2 отведениях ниже изолинии на 1 мм (см. рисунок). После 2-часового пребывания в накуренном помещении проведена повторная велоэргометрическая проба. Толерантность к нагрузке на этот раз составила 50 Вт, ДП - 200 усл. ед., ДП/Вт - 4.0. Причина прекращения велоэргометрической пробы - приступ стенокардии и смещение сегмента ST в 2 отведениях ниже изолинии на 1,5 мм.
Таким образом, у больного, страдающего стенокардией напряжения, III функционального класса, после "пассивного" курения уменьшилась толерантность к нагрузке, снизилось ДП и увеличилось ДП/Вт.

Subject E., 47 years old. Diagnosis: CHD; functional class III exertional angina. The patient presented with daily pressing retrosternal pain while walking or climbing stairs, which radiates out to his left arm. Pain abates within 2 to 3 minutes after exertion stops or after a dose of glyceryl trinitrate is taken. Coronary angiography revealed a 90% stenosis of the right coronary artery and a 50% stenosis of the circumflex branch of the left coronary artery. A cycle ergometry test was conducted on June 26, 1985. No baseline resting ECG abnormalities were found before testing.
Cycle ergometry conducted prior to passive smoking revealed an exercise tolerance value of 75W, a DP value of 220 conv. units and a DP/W ratio of 2.9. The exercise had to be terminated due to an onset of angina symptoms and the appearance of a 1mm ST-segment depression in 2 leads (Fig.). The test was repeated after 2 hours of exposure to passive smoke. Exercise tolerance, DP and DP/W values were 50W, 200 conv. units and 4.0, respectively. The exercise had to be discontinued because of an onset of angina symptoms and the appearance of a 1.5mm ST-segment depression in 2 leads.
Therefore, exposure to passive smoking in this functional class III exertional angina patient resulted in decreased exercise tolerance and DP values and increased DP/W ratio.

History: 10 years' BP treatment. Elevated BP despite medication, salt restriction and weight control. CHF 2 years ago; improved with digitalis and diuretics.
Physical exam: BP: 180/100 mm Hg in arms and legs. Ankle and lower leg edema. Fundus: Increased arteriole light reflexes, arteriovenous crossing compressions, no hemorrhages or exudates; flat disks. Apical impulse: enlarged, sustained, displaced to anterior axillary line. Normal S1; increased S2; S3 and S4 present. Rales at both lung bases.
Clinical studies: ECG: deep S wave in V2; normal R waves in V5 and V6. Low T waves in I, L, and V4 through V6. Chest roentgenogram: cardiomegaly; pulmonary vasculature prominent in upper lung fields. Serum electrolyte, BUN, creatinine levels, and urinalysis: normal. Echocardiogram: increased wall thickness; dilated LV cavity with global hypokinesis; 0.30 EF.

I first embarked on a career in translation while still a graduate biology student in 1998.
