Source text - Turkish Aritmiler, çocukluk döneminde erişkinlerden daha az sıklıkta
görülmektedir. Yakınması olmayan çocuklarda fizik muayene
sırasında tesadüfen saptanabilecekleri gibi göğüs ağrısı,
baş dönmesi, bayılma gibi belirtilerle de tanınabilmektedir 1,
2, 3.
Kalp hastalığı bulunmayan çocuklarda görülen aritmilerin
% 95’ini supraventriküler aritmiler oluşturmaktadır 4.
Ventriküler aritmiler daha az sıklıkta izlenmektedir. Uzun
QT ve Brugada gibi hastalıklarda kalbin elektriksel bozukluğuna
bağlı olarak ventriküler aritmiler normal kalplerde görülebilmektedir.
Viral miyokarditler, kardiyak tümörler, aritmojen
sağ ventrikül displazisi, kardiyomiyopatiler, ameliyat
edilmiş doğumsal kalp hastalıkları ventriküler aritmilere yol
açan diğer nedenleri oluşturmaktadır. Hipertrofik kardiyomiyopatiler,
Kawasaki ve doğumsal koroner arter hastalıklarında
iskemiye bağlı ventriküler aritmiler çocuklarda daha nadir
görülmektedir. Ani gelişen aritmilerde sıklıkla hızlı gelişen
elektrolit bozuklukları (hipokalemi, hiperkalsemi) ile hipoksemi
suçlanmaktadır 1, 3, 4.
Elektrokardiyografide saptanan aritminin bugüne kadar
yayınlanan ve bilinen patolojilerin hiçbirisine uygunluk göstermemesi,
ayrıca genellikle masum olarak kabul edilen antigribal
ilaçların kullanımında daha dikkatli olunmasını vurgulamak
için bu olgu sunulmuştur
Translation - English Arrhythmias are seen less in children in comparison to the adult population. These arrhythmias can be identified coincidentally during a physical examination of children with no complaints, as well as children suffering from nausea, dizziness and fainting 1,2,3 .
Supraventricular arrhythmias constitute 95% of the arrhythmias seen in children who do not have any heart disease 4.
Ventricular arrhythmias are seen less frequently. Ventricular arrhythmias can be seen in normal hearts due to a disorder of the heart’s electrical activity in the presence of diseases such as Long QT and Brugada. Viral myocarditis, cardiac tumors, arrhythmogenesis in right ventricular dysplasia cardiomyopathies, and other heart diseases which need surgery constitute other reasons which cause ventricular arrhythmogenesis. Coronary artery diseases such as hypertropic cardiomyopathy, Kawasaki and ventricular arrhythmias which are related to ischemia at birth are seen less in children. Frequent electrolyte disorders which may occur in rapidly developing arrhythmias (hypokalemia, hypercalcemia) and hypocalcemia have been identified as culprits1,2,4 .
This case is presented in order to draw attention to the careful use of anti-influenza drugs which are commonly accepted as harmless, and the lack of any resemblance of arrhythmogenesis seen in the electrocardiography to any of the previously published and known pathologies.
